    Reacción leucemoide asociada a sospecha de dengue sin signos de alarma, un abordaje epidemiologico

    Resumen: Presentación del caso. Lactante femenina de ocho meses, previamente sana, acudió por fiebre de tres días y eritema en miembros superiores y región perivulvar. Al examen físico se observó paciente irritable, febril al tacto, con mucosas semihúmedas y sin visceromegalias palpables. Ante el contexto epidemiológico, se consideró caso sospechoso de dengue sin signos de alarma, por lo que se indicó hidratación con cristaloides y tratamiento antipirético, decidiéndose su traslado a un hospital de segundo nivel. Intervención terapéutica. En el hospital de segundo nivel presentó diarrea y pobre ingesta oral; el hemograma evidenció anemia severa, leucocitosis y trombocitosis. Se hospitalizó e inició antibioticoterapia con ampicilina. Ante la persistencia de fiebre y deterioro hematológico, fue referida a un hospital de tercer nivel, donde se modificó el tratamiento a ceftriaxona y aciclovir intravenosos. Evolución clínica. Los estudios posteriores mostraron mejoría hematológica, normalización de leucocitos y aumento de hemoglobina. El frotis periférico evidenció microcitosis, hipocromía, células en tiro al blanco y equinocitos, hallazgos sugestivos de hemoglobinopatía. La paciente evolucionó favorablemente y fue dada de alta al tercer día de hospitalización, con indicación de realizar electroforesis de hemoglobina y seguimiento por hematología pediátrica. Abstract: Case presentation. An eight-month-old female infant, previously healthy, presented with a three-day history of fever and erythema on the upper limbs and perineal region. Physical examination revealed an irritable, febrile patient with slightly moist mucous membranes and no palpable visceromegaly. Given the local epidemiological context, the case was initially considered as suspected dengue without warning signs. Oral antipyretics and intravenous crystalloid hydration were administered, and the patient was referred to a secondary-level hospital for further evaluation. Treatment At that facility, diarrhea and poor oral intake were observed. Laboratory studies showed severe anemia, leukocytosis, and thrombocytosis, leading to hospital admission and initiation of intravenous ampicillin. Due to persistent fever and worsening hematologic parameters, the patient was transferred to a tertiary-level hospital, where treatment was modified to intravenous ceftriaxone and acyclovir. Outcome. Subsequent investigations revealed hematologic improvement, normalization of leukocyte count, and increased hemoglobin concentration. Peripheral blood smear demonstrated microcytosis, hypochromia, target cells, and echinocytes, findings suggestive of a possible hemoglobinopathy. The patient showed favorable clinical evolution and was discharged after three days of hospitalization, with a scheduled hemoglobin electrophoresis and pediatric hematology follow-up.
