dc.description.abstractLas enfermedades no transmisibles (ENT), especialmente la hipertensión arterial crónica (HTA-C) y la diabetes mellitus tipo 2 (DM-II), constituyen la principal causa de morbimortalidad en El Salvador. El presente informe final de trabajo de grado tuvo como objetivo mejorar la adherencia terapéutica en pacientes atendidos en la Unidad Comunitaria de Salud Familiar Intermedia “Francisco Antonio Lima” del distrito Jayaque, La Libertad Oeste, durante el período mayo-septiembre de 2025. Se diseñó e implementó una intervención educativa con 16 pacientes seleccionados aleatoriamente, mediante cinco charlas interactivas, entrega de material didáctico, promoción de hábitos saludables y seguimiento personalizado. La adherencia se midió mediante la escala Morisky-Green de 4 ítems antes y después de la intervención. Resultados: la adherencia inicial fue del 62,5 % y alcanzó el 87,5 % al final del proyecto, logrando una mejora estadísticamente significativa. El 100 % de los participantes reportó mayor conocimiento sobre las consecuencias de la no adherencia y estrategias prácticas de recordatorio. Se concluye que las intervenciones educativas grupales combinadas con seguimiento individualizado constituyen una estrategia eficaz, replicable y de bajo costo para mejorar la adherencia terapéutica en unidades comunitarias de primer nivel de atención. ABSTRACT: Noncommunicable diseases (NCDs), especially chronic arterial hypertension (C-HTN) and type 2 diabetes mellitus (T2DM), are the leading cause of morbidity and mortality in El Salvador. The present final undergraduate project report aimed to improve therapeutic adherence in patients treated at the Intermediate Community Family Health Unit “Francisco Antonio Lima” in the Jayaque district, West La Libertad, during the period from May to September 2025. An educational intervention was designed and implemented with 16 randomly selected patients through five interactive talks, distribution of educational materials, promotion of healthy habits, and personalized follow-up. Adherence was measured using the 4-item Morisky–Green scale before and after the intervention. Results: initial adherence was 62.5% and reached 87.5% at the end of the project, achieving a statistically significant improvement. One hundred percent of participants reported increased knowledge about the consequences of nonadherence and practical reminder strategies. It is concluded that group educational interventions combined with individualized follow-up constitute an effective, replicable, and low-cost strategy to improve therapeutic adherence in community health units at the primary care level.
